Forehead Lines
Horizontal lines across the forehead — and what drives them.
Forehead lines are among the most common concerns patients bring to consultation. They are the horizontal lines that run across the forehead — caused by the repeated upward movement of the frontalis muscle when we raise our eyebrows in surprise, curiosity or concentration. In youth, the skin recovers fully between expressions. Over time, as collagen and elastin decline, these repeated folds become etched into the skin and remain visible even at rest.
Forehead lines are almost always dynamic in origin — driven by muscle movement rather than volume loss or skin laxity. This makes them one of the most straightforward concerns to treat effectively. The challenge is in treating them well — using the right amount of product in the right locations to soften the lines without flattening the forehead entirely or affecting the natural movement of the brow.
The frontalis is the only muscle that lifts the brow. Overtreating it can cause the brow to drop, making the eyes appear heavier. This is why forehead lines should always be treated in the context of the whole upper face — assessing brow position, the strength of the frown muscles, and the patient’s natural expression before deciding on the approach.
Treatments to consider.
Anti-wrinkle injections are the primary and most effective treatment for forehead lines. Precise, low doses of botulinum toxin relax the frontalis muscle, softening the lines while preserving natural movement and brow position. Results typically last three to four months and improve with regular treatment over time as the muscle gradually weakens with consistent relaxation.
For lines that remain visible at rest even after anti-wrinkle treatment — indicating a deeper, more established crease — a small amount of dermal filler can smooth the residual line. Skin boosters and professional skincare improve skin quality and resilience, making skin less prone to creasing and supporting the results of clinical treatment between sessions.
