Downturned Mouth

When your resting expression doesn’t reflect how you feel.

A downturned mouth is one of those concerns that patients often describe in the same way: people ask if they are tired, sad, or cross when they are not. The corners of the mouth pull downward at rest, creating an expression that can read as negative or disapproving regardless of mood. It is a concern that tends to worsen gradually with age — and one that responds very well to treatment.

A downturned mouth is usually the result of a combination of factors. The depressor anguli oris muscles — which run from the corners of the mouth downward — can become overactive with age, actively pulling the corners down. At the same time, volume loss in the lower face reduces the structural support that keeps the corners lifted. Skin laxity can also contribute as the soft tissue around the mouth loses firmness over time.

Treatments to consider.

The approach depends on what is driving the concern — muscle activity, volume loss, or a combination of both. Small doses of botulinum toxin placed into the depressor anguli oris muscles relax the downward pull and allow the corners of the mouth to sit more naturally. Dermal fillers restore volume and support to the corners of the mouth and the surrounding area, lifting the corners and improving the overall appearance of the lower face. In many cases a combination of both produces the most natural and complete result.
